Recently we have received questions regarding when to bill using CDT code D7140 versus D7111. The differentiator is whether the root is in bone. If the root is completely in soft tissue, the correct code to use is D7111.

When a claim is received for D7140, if the tooth in question is within the average exfoliation period for the child (based on age), an x-ray is requested for review. Please be advised that if the tooth's root structure is not clearly visible in bone on the x-ray, you may also submit a photo, at your own cost, if you feel that will assist the dentist reviewing the case in making a clinical determination. Requirements for Oral Surgery Referrals

When you submit a referral request to MCNA for the evaluation of third molars (1, 16, 17, 32) by an oral surgeon, it needs to be accompanied by specific information. The following required information allows our Utilization Management staff to most effectively evaluate the referral:

  • Narrative or office remarks: these should note the tooth numbers and concisely describe the symptoms
  • X-rays: these can be pano, periapical, or bitewings

If you submit a request for a referral without also submitting a narrative and an x-ray, it will not be approved. Please note that MCNA will not authorize the removal of asymptomatic third molars as there is no benefit for the extraction of asymptomatic teeth that show no signs of infection. This includes but is not limited to the removal of third molars.
