Requests for MCNA's reconsideration of a claim may be filed when a claim has been denied for anything other than medical necessity or benefit coverage including, but not limited to, the following examples:
Any supporting documentation should be included with the reconsideration requests. Providers are encouraged to submit their request online using MCNA's Provider Portal (http://portal.mcna.net). Once you have logged into the Provider Portal, please click on "Support and Downloads" to access the Online Reconsideration/Appeal link. Please complete the electronic form titled "Provider Reconsideration and Appeal" form including all information needed to evaluate your request. More information including requirements and time frames for the reconsideration process can be found in MCNA's Provider Manual, which is always available to download from our Provider Portal or website. You may also call MCNA's Provider Hotline at 1-855-776-6262 (8 a.m. to 7 p.m., Central Time, Monday through Friday) if you have questions. |
