Keeping MCNA updated with your dental practice information when changes occur is important to both you and your patients. When you report practice changes, we are able to relay correct information to our members regarding your location, hours of operation, and contact information to make an appointment. Accurate practice information also assists us in processing your claims without unnecessary delay.

Is Your Practice Information Up-to-Date?

Keeping MCNA updated with your dental practice information when changes occur is important to both you and your patients. When you report practice changes, we are able to relay correct information to our members regarding your location, hours of operation, and contact information to make an appointment. Accurate practice information also assists us in processing your claims without unnecessary delay.

As a best practice, please notify MCNA by calling our Provider Hotline whenever there is a change in any of the following information:

  • Name - Name of a dentist, name of the practice/clinic, or the DBA name of practice
  • License - License number (please provide the full license number of treating dentist as issued rather than a shortened version)
  • Location - Newly added location, or when a dentist changes or ends a location
  • National Provider Identifier (NPI) - Individual NPI as well as the practice/clinic NPI
  • Tax Identification Number (TIN) - Completed W-9 along with note requesting TIN change, effective date of TIN change, and any other associated office changes
  • Adding/Removing Associates - Newly added associate, or when an associate retires or otherwise leaves the practice
  • Ownership Change - New clinic name (if applicable), date of purchase, the TIN (if not currently on file), and a list of all dentists associated with the TIN
